Returns, Missing Items and
Terms and Conditions:
Due to the Food Drug
Administration (FDA) regulations, once any product is open or
taken out of its original packaging, it is no longer returnable.
Therefore in order to comply with these regulations, if either
any equipment or any supplies have been open and/or used, the
item(s) is no longer returnable – there will be no exceptions.
If the product is defective,
and you have the original packaging, contact us within the
manufacturer’s warranty period to obtain a return authorization
number. We will then exchange the product pursuant to the
manufacturer’s warranty. No substitutions are allowed. All
warranty decisions are made by the manufacturer and are

the equipment is non-defective and unopened; additionally
returns will not be accepted without prior written
authorization. You MUST contact us within 15 days of the
purchase date to receive a return authorization number. Any item
returned without prior authorization or after 15 days will NOT
be credited and will only be returned at the owner’s expense –
there will be no exceptions.
Any and all authorized returns
are subject to a 20% restocking fee (applicable once order has
left our location). Shipping charges are non-refundable.
If a product is damaged in
transit you must contact our office within 3 days of receipt, in
writing, describing the damage. All packaging must be retained
in order for us to replace transit damaged goods. We will only
replace transit damaged merchandise with the same merchandise.
No substitutions are allowed. Under NO circumstances will
transit damaged merchandise be replaced if the 3 day written
notification or 3 days receipt period has passed – there will be
no exceptions .
All return correspondences should be submitted either by e-mail
to email@example.com or by US mail to Sleep Restfully,
Inc. at 3100 Weslayan St., Suite 373 Street, Houston, Texas 77027. All product
returns should only be sent after receiving authorization from
SleepRestfully.com located at 3100 Weslayan St., Suite 373, Houston,
Texas 77027 and MUST include the return authorization number on
the outside of the article.
When you purchase an item
through Sleep Restfully, Inc. please be advised we do NOT accept
any insurance. Payment on any items purchased is due at the time
of the purchase. Sleep Restfully, Inc. is not a participating
provider of any insurance plan and will not bill your insurance
provider. Sleep Restfully, Inc. will provide you with the codes
of the items you purchased for you to submit to any insurance
provider you may have. Sleep Restfully, Inc. does NOT make any
representations, expressed or implied, as to whether the items
purchased are covered under any insurance provider you may have;
this includes but is not limited to Medicare, Medicaid or any
other third party insurance payor.
Additionally, when you purchase
an item through Sleep Restfully, Inc. you acknowledge that you
have discussed this matter with your physician and the item(s)
being purchased is the item you require based upon those
discussions. You also acknowledge that any item(s) you purchase
is the specific item(s) you desire.

State of Texas, applicable to agreements made and entirely to be
performed within such state, without resort to its conflict of
law provisions. By accessing and/or placing orders under our
websites, you acknowledge that you have read, understood and
laws and regulations, including U.S. Export control laws and
should not access our websites. You agree that any action at law
shall be filed only in the State of Texas located in State or
Federal Court and you hereby irrevocably and unconditionally
consent and submit to the exclusive jurisdiction of such courts
over any suit, action or proceeding arising out of these Terms
