The FDA requires that we have a written
prescription from a US licensed physician in order to provide you with a CPAP or BiLevel machines stating the
pressure the machine is to be set for. You must also have a
prescription for mask. Please fax your
prescription to (800) 525-4293 or e-mail a scanned copy to
sales@sleeprestfully.com. Your prescription should have the
physician's information and signature, the type of machine you
need (CPAP, Auto CPAP, or BiLevel), and the pressure at which
the machine is to be set. Your doctor may also want include what
type of humidification is recommended but this is not necessary.
If you do not have a written prescription we will be happy to
contact your prescribing physician to obtain one on your
behalf. In this case please fax us at (800) 525-4293, give
us all physician contact information on the customer checkout
form in the "Additional Information" area or e-mail
your prescribing physician’s name, address and phone number to
us at
sales@sleeprestfully.com.
No merchandise requiring a physician's prescription will be
shipped prior to Positive Air, Inc. obtaining one.
Prescriptions are also required for all respiratory
equipment as well as any equipment which we will be billing
Medicare, Medicaid or any insurance carrier.
